Job Description

RESIDENTIAL ADVISORS

Horizon House is seeking Residential Advisors with experience working with medically fragile individuals in a residential/community setting in Philadelphia County. Residential Advisor provides support with daily living activities, community integration, medical appointments, religious events, and the development of Individual Service Plans (ISP) to individuals experiencing Mental Health/Intellectual Disabilities diagnosis. A Residential Advisor will find assisting individuals with learning to live and function as independently as possible in the community rewarding. FT / PT / ON-CALL positions and various shifts available.

Qualifications:

  • A.A. degree in the social sciences or health care related field and one year of relevant experience in the MH/MR field; prior experience with implementing behavior management plans and/or supports; or an combination of experience, education or training that would provide the level of knowledge, skill and ability required.
  • Good communication skills, both oral and written.
  • Ability to perform independently and as a team member.
  • Ability to use a computer and document daily activities electronically.
  • Required to possess a valid PA driver's license and maintain an acceptable driving record.

 

REHABILITATION COUNSELORS

This position provides care and supervision of all assigned residents to ensure a safe, healthy, secure and pleasant environment, which promotes growth and development; facilitates client growth and development toward independence in self-care, daily living skills, socialization and community living; participate in the development of the individual service plans and related documenta­tion and document client progress as they move through the service delivery system. FT/PT/PER DEIM positions and various shifts available.

Qualifications:

  • A.A. degree/60 credits in the social sciences or health care related field and one year of relevant experience in the MH/MR field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
  • Demonstrated crises intervention/prevention and de-escalation skills required.
  • Good communication skills, both oral and written.
  • Possess valid driver’s license with acceptable driving record.
  • Ability to work independently and as part of a team.
